Wright Medical Center's Health Information Management Department is responsible for maintaining a medical record on every patient.  Medical Records exist in paper and electronic documentation formats.  HIPAA, federal and state laws, determine how personal health information is accessed and used.

You have a right to request copies of your medical record.  To obtain copies you are required to complete an authorization form.  You may call or write the Health Information areas in the hospital or clinic to obtain the form.
